2014 IATCCC Track Clinic and Membership
MAKE SURE YOU TYPE IN YOUR EMAIL CORRECTLY IN ORDER TO GET A RECEIPT!!
BE SURE TO COMPLETE ALL 4 STEPS TO SUCCESSFULLY REGISTER, PLEASE DO NOT PRINT AND FILL IN THE FORM TO MAIL, THIS DOES NOT REQUIRE PAYMENT ONLINE
1. Click Register on the right OR to use a previously created profile sign in and register
2. Complete the registration form, then click Continue
3. Review your registration information or add other attendees, then click Continue
4. Choose your payment option (Credit Card, Checking Account, or Pay Later), then click Submit Payment.
a. TO PAY BY MAILING A CHECK select Pay Later
b. If you choose to pay by mailing a check,
payment must be postmarked by Friday, Jan 24, 2014

5. YOU MUST COMPLETE ALL FOUR STEPS AND
GET A CONFIRMATION NUMBER TO BE REGISTERED
YOU WILL SEE A NICE GREEN BOX THAT STATES YOU HAVE SUCCESSFULLY REGISTERED
You will be emailed a receipt with a confirmation number
when you have completed all 4 steps. Check that you got
the email before closing the page with the green box on it.
If for some reason you do not get an email, print or save
the webpage with your confirmation
You must sign up at least 5 people at once and make one payment
If you are paying with a credit enter that persons name last.
If you are mailing payment it must be one check.
If you cannot make it to the clinic, but still want to become a Member for 2014
